Organizations may also apply for volunteer support, donations, and request the presence of speakers on selected health topics. Requests for volunteers, speakers, or in-kind donations do not require an application, and will be accepted on an ongoing basis through October 31, 2008.
Requests for volunteers, speakers, and in-kind donations (e.g. equipment, supplies, furniture, etc.), must be submitted on the organization’s letterhead (not to exceed 2-pages) with a copy of your organization’s/fiscal sponsor’s letterhead, and a copy of current IRS determination letter indicating 501 c (3) tax-exempt status. Requests must be submitted at least one month in advance. Depending on the request, your letter should include the following information:
- Description of the organization, its mission and values, area of focus, services provided and target population
- Description of the request, including dates, duration of the activity, and location, or
- Description of the donation being requested, and a brief explanation on how the equipment, supplies, and/or furniture will be used
